Shouldn't happen again, but then again I have no idea how it got into a blocked wall, so what do I know.

City of the Dead raid made the square traversable, and then the Ancient was never killed before it became non-traversable again.

I think this is the same bug or closely related. Both involve loot stuck in a wall after a raid has ended.

How could a silver bar get stuck in the catacombs S or N wall? They only spawn in the raid.
