From what I recall, any infraction makes it possible for a Vampire to be attacked at sunrise, but that the hunter attacks are random. I know with Quoth, the only time he was attacked by day time hunters was in the couple days immediately following failed feeding. (Sec. 3 or higher) Personally I dont see why a failed feeding in a sec 2. would result in attacks given that they have no bearing on the city alert level.
Then again... with all this contradictory information, it looks more likely that any Masquerade/Notoriety breech makes a vampire eligible to be attacked, with the determining factor being a random dice roll.

Well the city alert has a lot to do with how often they happen.
But as far as WHO gets attacked? If it's just random... I'll eat my hat.
There have to be some factors that add or subtract to the likelihood of one vampire being a target over another.

Why? Hasn't one of the main premises of path been randomness to emulate reality? A person who has never fired a gun before can get a lucky shot, just like a person can be punished for the misdeeds of others. Now don't get me wrong... I think the more violations a vampire has, the more likely it is they will get hit, but I also think the likelihood is affected far more by random chance.

By random roll....
Just as pure example lets to get attacked by hunters an one hundred sided dice is rolled. To be attacked the roll out come has be lets say eighty or higher.
In this example one infraction would qualify you to be rolled upon, and each additional infraction/ point of notoriety would add +1 to the roll out come. Even if Wolffyn racked up fifty violations (+fifty to the roll) if every morning at sun up her random roll was below twenty nine, she still wouldn't get attacked.

Notoriety
Just like in the real world, undertaking certain actions will attract police attention. Each action (listed below) that garners notoriety will increase your chances of being recognized by police officers.
Cops have an X in 30 chance of recognizing you, where X is the notoriety points. Notoriety points can go up to 60, so cops may have a 100% chance of recognizing you if you walk by. When you are noticed by a cop, they will attack. Whether or not you are wounded depends on your stats.
Notoriety points are reduced by 3 each day.
- Actions that increase Notoriety:
- Failing a hack
- Killing Cops
- Killing Gangsters
- Killing Guards during breaking and entering
- Being spotted feeding in high security areas
I believe Hunters are also drawn to those with increased notoriety. BnE peeps are more apt to be targeted cause they are more likely to kill multiple guards/cops each day. There is also the high alert thing, making everyone a target with a big bowl of randomness.
Sun gets hit at least 1-2 times a week, which is the price of doing business.

First of all I should start by saying that ANY vampire is a viable and possible target for hunters, especially if the city alert is high. The higher the alert goes, the more 'innocent' vampires will get hit, which makes even non-violators care. However, vampires who have most recently drawn attention to themselves or exposed themselves as vampires to humans are most at risk.
a) All of them. But not only secrecy violations. Killing cops and guards is going to lead to investigations, which is going to lead to more attention, which occasionally attracts the attention of hunters.
b) If you lay low after a period of being spotted feeding / using powers / killing cops (etc.), then your chances of being hit are going to drop much lower after a week or two.
c) Yes. The more human NPCs who see you do something, the more chance that a hunter is going to hit you. Also, killing cops outside of a B&E event is one of the worst things you can do if you want to avoid hunter attacks. But even inside B&Es, killing innocents is a bad idea, so try to sneak.
d) You can hack to find evidence of your violations stored on human computers, and remove this incriminating evidence. I believe we were planning on adding a ritual which also does a similar thing (removes memories from random NPC humans to drop your chances of being exposed), though we haven't got around to it yet. I don't believe there's anything else right now, though.
