Didn't see any re-spawns for a little while but yesterday (9/1) and today (9/2) I've had Hunter Footsoldiers respawn in the same square... though on both occasions it was only one respawn. Can't really say for certain if it is out of line with the 100-1000 ratio we should be expecting.

Was it just once? It's possible that there's still a location out there that's causing them to not respawn properly. Long story.

Yes, all four times it happened, the NPC spawned in that location only once more.

Are the locations that they are happening in part of the problem or something we should track?

No it's nothing you guys can do. I'll have to figure out which location is set up wrong, as that's the one (or two) which is most likely causing them not to spawn elsewhere.

Okay I think I've found all the offending locations now (which took about an hour, lol). Keep reporting it if it seems like it's happening too often, though.

should we state where it was on grid when/if it happens again?

Just had a fadebeast respawn in the same location 3 times in a row (11, 13 High Noon Saloon)

How often is this still happening? I know there must be a location somewhere which is causing a non-respawn on kill (not the location you find them in, but a square which is essentially void, causing them not to move). The priority for fixing this would depend on how often it happens.
